29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. Maintenance Shop - On or about August 1, 2013, the unused portion of the blade of the Delta, vertical band saw, Model 0712009, serial # Plan 31, was not enclosed or guarded, thereby exposing employees to the hazards associated with reciprocating blades.    b. Maintenance Shop - On or about August 1, 2013, the unused portion of the blade of the horizontal Turnpro band saw, Model 0712011, was not enclosed or guarded, thereby exposing employees to the hazards associated with reciprocating blades.    c. Maintenance Shop - On or about August 1, 2013, the employer failed to guard the Turnpro lathe chuck, serial # 0200919873, thereby exposing employees to the hazards associated with rotating parts.   d. Kitchen - On or about August 1, 2013, the employer failed to guard the knives on the pasta trimmer, thereby exposing employees to laceration and amputation type injuries.

Kitchen - On or about August 1, 2013, the employer did not have the points of operations completely guarded, in that the safety lid on the Dominioni Macchine Pasta Fresca mixer was open, bypassing the safety device, thereby exposing employees to rotating parts.  c. Kitchen - On or about August 1, 2013, the employer did not have the points of operations completely guarded, in that the safety device on the lid of the pasta roller was bypassed with shrink wrap, thereby exposing employees to the points of operations.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  Throughout Facility - On or about August 1, 2013, personal lockout or tagout devices were not attached to a group lockout device when two employees were sanitizing or repairing the ovens and freezer tunnels, thereby exposing employees to the hazards of the unexpected re-energization of equipment.  American Kitchen Delights, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(f)(3)(ii)[D], which was contained in OSHA inspection number 312592504, citation number 1, item number 2 and was affirmed as a final order on June 15, 2009, with respect to a workplace located at 15320 S. Cooper Avenue, Harvey, Illinois 60426.
