29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) The written energy control (lockout) program was inadequate in that it did not include provisions for periodic or at least annual inspections of the energy control procedures with records kept. (b) The machine specific energy control (lockout) procedures for palletizers and de-palletizers did not include a raised machine component element (pallet elevators) and gravity as potential sources and the control methods.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees: (a) An employee on the #1 palletizer line was not trained as an authorized employee to lockout to perform tasks, such as, but not limited to, clearing blocked eyes, entering the palletizer to pick up dropped bottles, and retrieving and replacing flags on the conveyor line stops that had fallen off.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: (a) There had not been any periodic or at least annual inspections of the energy control (lockout) procedures since August 23, 2011 and the procedures reviewed then were inadequate in that the written procedures developed on 5/26/2010 did not include gravity and stored energy sources on the equipment such as, but not limited to, a raised pallet elevator on the #1 bottle palletizer. G & J Pepsi-Cola Bottlers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 313520066, citation number 1, item number 2, and was affirmed as a final order on 10/20/2009, with respect to a workplace located at 2580 Bobmeyer Rd., Hamilton, OH. 45015.
