29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.    a) 1003 Tumblestone Drive, Mount Joy, Pa -  A laborer was observed operating a DeWalt nail gun to secure trim boards and was not wearing eye protection, as referenced in 1926.102(a)(1), thereby exposing the employee to possible eye injuries, on or about August 1, 2013.    b) 1003 Tumblestone Drive, Mount Joy, Pa - A laborer was observed operating a Makita circular saw to cut siding and was not wearing eye protection, as referenced in 1926.102(a)(1), thereby exposing the employee to possible eye injuries, on or about August 1, 2013.    No abatement certification or documentation required.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user.    a) 1003 Tumblestone Drive, Mount Joy, Pa - A laborer was observed working from a pump jack scaffold which had intermixed scaffolding components used to extend the length of the working platform and was exposed to a 18 foot plus 2-1/2 inch fall hazard to the ground below, on or about August 1, 2013. No abatement certification or documentation required.    S & M Siding was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii) which was contained in OSHA inspection number 315815837, Citation number 2, Item number 2(a), and was affirmed as a final order on 2/24/12, with respect to a workplace located at W. King Road & Griffith Avenue, Malvern, Pa 19355.    S & M Siding was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.451(g)(1)(vii) which was contained in OSHA inspection number 314239658, Citation number 1, Item number 3, and was affirmed as a final order on 9/27/10, with respect to a workplace located at Tumblestone Drive, Mount Joy, Pa 17552.
