On or about August 1, 2013, at the above addressed jobsite, employees removing the various parts from beneath the scrapped vehicles were exposed to head injuries due to the lack of head protection.

29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5):  a. On or about July 31, 2013, at the above addressed jobsite, employees operating the fork lift trucks as well as the Volvo 70E wheel loaders which were equipped with quick attachment units for the lifting forks, had not received training in the safe operation of the equipment. The quick attachment units on the Volvo 70E wheel loaders allowed the operator to connect to the forks from the operator's seat but training was not provided as to the procedure to assure that the hydraulic pins had engaged to hold the fork attachment onto the factory mounted support bracket, thereby exposing employees to struck by hazards.

29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty:  a. On or about July 31, 2013, at the above addressed jobsite, an employee received fatal injuries after being crushed beneath a vehicle which had been supported on the quick attached forks of a Volvo 70E wheel loader. The hydraulic locking pins, which secured the quick attached forks to the support bracket of the wheel loader, had not engaged allowing the forks and the vehicle to fall. Employees were thereby exposed to struck by hazards.
