29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about July, 31, 2013, at the Park Avenue Apartments located at 308 South University Ave, Little Rock, AR 72205, the employer did not ensure employees were properly wearing personal fall arrest systems while working in an aerial lift. Employees were observed not attaching the snap-hook of the personal fall arrest system to the boom or basket, exposing employees to fall hazards of greater than 22 feet to unfinished ground below.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about July 31, 2013, at the Park Avenue Apartments in Little Rock, AR, the employer did not ensure employees properly wore their personal fall arrest systems. The D-ring attached on retractable harness worn by each, were located below the center of wearer's back. The employees were exposed to fall hazards from an elevation greater than 22 feet to the unfinished ground level.
