29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes: a) In the Work Room - On or about September 12, 2013, the employer had not provided employees performing housekeeping operations on asbestos-containing material with an asbestos awareness training course, covering the physical and health hazards associated with asbestos exposure, locations of ACM and PACM in the building/facility, recognition of ACM and PACM damage and deterioration, and proper response to fiber release episodes, at least annually. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) In the Work Room - On or about September 12, 2013, the employer had not provided employees with effective information and training on the measures employees could take to protect themselves from the chemical hazards, along with the specific procedures and work practices the employer had implemented to protect employees from exposure to hazardous chemicals, including but not limited to floor wax stripper, tile floor wax, wax sealer, and the material that could be evolved from the floor tiles if they were too vigorously maintained or severely ill-treated and allowed to become porous. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1001(d)(2)(i): The employer did not perform initial monitoring of employees who were, or might reasonably be expected to be exposed to airborne concentrations at or above the 8-hour TWA and/or excursion limits: a) In the Work Room - On or about September 12, 2013, the employer, assigning housekeeping tasks involving known asbestos-containing material, had not monitored each of the asbestos-related housekeeping work operations to accurately determine the airborne concentrations of asbestos to which employees may have been exposed. No abatement certification or documentation required.
