29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met (Construction Reference: 1926.59):   a) At the site, where the employees used Rector Seal PVC sealer, containing methyl ethyl ketone, to join the PVC piping. There was no hazard communication program maintained at the site.

29 CFR 1910.1200(g)(8): The employer did not maintain copies of the required safety data sheets (SDS) for each hazardous chemical in the workplace (Construction Reference: 1926.59):   a) At the site, where the employees used Rector Seal PVC sealer, containing methyl ethyl ketone, to join the PVC piping. There was no safety data sheet for the Rector Seal PVC maintained at the site.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided:   a) At the site, where the employees were installing a new sewer line inside of an excavation. The spoil piles were not maintained at least two feet away from the excavation edge. This condition exposed employees to a struck-by hazard.   b) At the site, where the employees were installing a new sewer line inside of an excavation. The excavator was not maintained at least two feet away from the excavation edge. This condition exposed employees to a struck-by hazard.

This condition exposed employees to a caught-in hazard.
