29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: On or about July 31, 2013, at Park Avenue Apartments located at 308 South University Ave, Little Rock, AR 72205, an employee working on scaffolding was exposed to head impact hazards from stacked bricks and other construction materials.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. On or about July 31, 2013, at Park Avenue Apartments located at 308 South University Ave, Little Rock, AR 72205, an employee was working on scaffolding with no proper access. The employee was exposed to fall hazards while climbing the cross braces to access the work area.

29 CFR 1926.451(c)(1)(i): Guys, ties, and braces were not installed at locations where horizontal members support both inner and outer legs: The Metroplex Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(c)(1)(ii), which was contained in OSHA inspection number310447057, citation number 1, item number1 and was affirmed as a final order on 02/09/2009, with respect to a workplace located at 8301 Monterra Drive, Fort Worth, TX 76137. On or about 31 July 2013, at Park Avenue Apartments located at 308 South University Avenue, Little Rock, AR 72205 an employee was working non a scaffolding that was not missing cross-bracing in several locations throughout the scaffolding. As a result employees were exposed to fall and scaffold collapse hazards.
