29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about July 30, 2013, and in times prior thereto, the employer failed to train employee(s)in the hazards associated with the use of portable generators and other equipment. An employee used a gas fueled portable generator inside a 20 ft. mobile container with a combination office/storage area to provide power to the unit, exposing the employee to the hazards of carbon monoxide inhalation.
