29 CFR 1910.305(g)(2)(iii):  Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:               This violation most recently occurred on August 02, 2013, in the fabrication area, next to the laser cutting machine, where an approximate 50-foot electrical extension cord that powered a fan had separation of the outer insulation from the plug, exposing the conductors to strain.
