29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about August 2, 2013, employees were engaged in decking activities on a steep roof at a height greater than six (6) feet above ground level without using any method of fall protection, exposing the employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about August 2, 2013, employees engaged in decking activities at a height greater than six (6) feet above ground level were not trained to recognize fall hazards nor trained in the procedures to be followed in order to minimize fall hazards.
