On or about August 4, 2013, at the above addressed job site, employees were exposed to a struck /by hazard while working in proximity to a crane that had been used with an outrigger pad that had sunken into the ground due to insufficient support materials.

On or about August 4, 2013, at the above addressed job site, employees were exposed to a fall hazard while removing roofing materials from a second story roof without a means of conventional fall protection.
