29 CFR 1910.132(c): Personal protective equipment was not of safe design and construction for the work to be performed: a) I-395 Waterford Job Site, southbound side just north of exit 77 - On or about 8-2-13 - Employee working in the articulating aerial bucket was not using the proper lanyard to protect the employee during a fall from contacting a lower level or surface.

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: a) I-395, Waterford Job Site, southbound side just north of exit 77 - On or about 8-2-13 - Employee was trimming trees and dicing limbs with a Husqvarna chain saw that had a broken chain brake. Employee was severely cut on the neck, jaw and shoulder when the saw kicked back on him and the chain brake did not function.
