29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. (a) South side, new residential construction located at 600 Concord Lake Lane, Smyrna, GA 30082: Employee was exposed to a collaspe/fall hazard while working on a 24 foot pump jack scaffold platform with a makeshift 2x10 attached to the platform with a 2 inch S-Hook ratchet strap which extended approximately 5-feet, 9-inches past the Alum-A-Pole support pole.

(a) West side, new residential construction located at 600 Concord Lake Lane, Smyrna, GA 30082: Employee was exposed to falls of approximately 13-feet, 2-inches while installing Hardie Plank Lap Siding on a pump jack scaffold system without a fall protection system in place.
