Employee was exposed to falls to the floor below as the employee when using the multiladder in the twin stepladder position was straddling the front and rear sections and standing on the second rung from the top of the ladder, both actions prohibited from the manufacturer, Werner Co. The employer had not provided retraining to this employee on ladder safety specifically relating to the use of the Werner Multiladder model. Violation observed on or about 08/02/2013.
