29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about July 3, 2013, at a job site located at 315 E Dunklin Street in Jefferson City, MO, an employee, who was not wearing a hard hat, was struck and injured when concrete form panels stacked on a flatbed trailer became dislodged and fell on him.

29 CFR 1926.602(d): The employer did not certify that each powered industrial truck operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l) as set forth by the Note at the end of 29 CFR 1926.602(d): a) On or about July 3, 2013, at a job site located at 315 E Dunklin Street in Jefferson City, MO, an employee was operating a track front end loader with a forklift blade attachment unloading concrete forms from a flat bed trailer. The operator had not been trained and certified in the operation of the device as required by standard.

29 CFR 1926.651(e): Employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials: a) On or about July 3, 2013, at a job site located at 315 E Dunklin Street in Jefferson City, MO, an employee was working in close proximity to a flatbed trailer while a front end loader with a blade attachment was unloading 90 lb. concrete form panels. Several of the panels became dislodged and fell on the employee.
