29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Clarion Elizabeth Logistics Center - Next to 623 Dowd Avenue, Elizabeth, NJ 07201 - Employees were exposed to fall hazards of approximately 40 feet from the ground below during the cutting of holes in the roof for skylight installation without fall protection. Condition occurred on or about August 5, 2013. Wolfe Roofing and Sheetmetal Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 112984935, citation number 1, item number 1 and was affirmed as a final order on 09/14/2009, with respect to a workplace located at 5900 Perkiomen Avenue, Reading, PA.

Condition occurred on or about August 5, 2013.
