(a) Location - Facility: On or about 8/2/2012, a Nissan forklift had been modified to lift and move molds for injection molders. The modification consisting of an eye bolt and a hook that were attached to a single fork by two lock nuts had not been approved by the manufacturer of the forklift.

(b) Location - Facility: On or about 8/5/2012, a Nissan forklift was not in safe operating condition in that the carbon monoxide (CO) levels 12 to 18 inches from the exhaust exceeded 1000 parts per million (ppm).
