29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:  (a)  An open-sided platform on top of shredder No. 4 did not have any type of fall protection in place, exposing employees to fall hazards of 7 feet  3 inches onto a concrete floor on ground level.     On or about 7/19/13, an employee was injured after falling from the unguarded work platform on top of shredder No. 4 while removing a shaft.  The employee suffered a broken vertebra(e)  in the lower back.         NO ABATEMENT CERTIFICATION OR DOCUMENTATION REQUIRED
