29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, 1301 Beaver Avenue, Pittsburgh, PA: On or about July 16, 2013, during pier cap construction activities, the employer did not ensure that fall protection system/devices, such as guardrails, were used along the end of a form scaffold, exposing employees to fall hazards of up to approximately 30 feet as they worked to assemble a rebar system.
