29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) Roof of building, on or about August 5, 2013: Guard rail system was installed with fasteners that were not appropriate for the installation, in that incorrect type and size of fastener was used. The employer did not train the employees in the correct procedure for erecting the guard rail receivers using the proper fasteners as recommended by the manufacturer.
