29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:     a) On or about 08/06/13 - Facility, a PECO saw operator was exposed to continuous noise levels  at 69.92 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 87.4 dBA. The sampling was conducted for 469 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (11 minutes).     b) On or about 08/06/13 - Facility, a PECO saw operator was exposed to continuous noise levels  at 110.3 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 90.7 dBA. The sampling was conducted for 470 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (10 minutes).     c) On or about 08/06/13 - Facility, a GBN pallet nailing machine operator was exposed to continuous noise levels  at 79.59 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 88.3 dBA. The sampling was conducted for 435 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (45 minutes).      d) On or about 08/06/13 - Facility, a Brewco resaw operator was exposed to continuous noise levels  at 71.81 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 87.6 dBA. The sampling was conducted for 446 minutes on 8/7/13.  Zero exposure was assumed for the remainder of the shift (34 minutes).     e) On or about 08/06/13 - Facility, a Brewco machine feeder was exposed to continuous noise levels at 71.81 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 87.6 dBA.  The sampling was conducted for 457 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (23 minutes).     f) On or about 08/06/13 - Facility, a GBN pallet nailing machine operator was exposed to continuous noise levels  at 105.5 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 90.3 dBA. The sampling was conducted for 471 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (9 minutes).     g) On or about 08/06/13 - Facility, a GBN pallet nailing machine feeder was exposed to continuous noise levels  at 139.5 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of approximately 92.4 dBA. The sampling was conducted for 472 minutes on 08/07/13.  Zero exposure was assumed for the remainder of the shift (8 minutes).
