29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can walk shall be guarded by a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing. Employees working in the boiler room were exposed to fall and tripping hazards in that the 16 in. x 24 in. x 17 in. deep floor opening located in the corner of the room near the overhead door was not guarded.

29 CFR 1910.1030(d)(4)(iii)(A)(2)(i): Sharps containers shall be easily accessible to personnel and located as close as is feasible to the immediate area where sharps are used or can be reasonably anticipated to be found (e.g., laundries); Employees were exposed to needle sticks with contaminated sharps in that a sharps container was not located in the laundry room.
