29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can walk shall be guarded by a floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.  Employees working in the boiler room were exposed to fall and tripping hazards in that the 16 in. x 24 in. x 17 in.deep floor opening located in the corner of the room near the overhead door was not guarded.
