29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about August 5, 2013, the eye wash servicing the work area above the mud pit was not available for immediate use. The eye wash was missing the cap and the eye flushing solution was contaminated with dirt and other debris.

29 CFR 1910.184(e)(1):Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: On or about August 05, 2013, and at times prior thereto, the employer did not ensure that all of the alloy steel chains used on Unit EB#1H had identification tags that were legible and installed on the slings, exposing employees to being crushed by falling loads in the event of overloading the chain.
