29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: A. Reinbolt Siding LLC worksite located at 536 Jamestown Ave., Tiffin, Ohio: On or about August 6, 2013, the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns when performing weatherproofing activities on the north side of the two story residential structure. The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.

Reinbolt Siding LLC worksite located at 536 Jamestown Ave., Tiffin, Ohio: On or about August 6, 2013, the employer did not ensure employees were protected from falls while installing weatherproofing on the South side of the two story residential structure. The employees were exposed to a potential fall distance of approximately 8.00 feet.
