29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed: a) On or about August 6, 2013, at the above located workplace, employees were required to work in a warehouse. The warehouse's southwest emergency exit route was blocked with copy machines and other material. Employees were exposed to the hazards associated blocked exits during emergencies (fire, smoke inhalation).

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a) On or about August 6, 2013, at the above located workplace, employees were required to operate powered industrial trucks. The employer had not ensured that each powered industrial truck operator had completed training and was evaluated on the safe operation of powered industrial trucks. Employees were thereby potentially exposed to the hazards associated with powered industrial trucks.
