29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  On or about August 6, 2013, the employer did not provide training that prohibited employees from standing on the guardrails of the Genie G1930 Scissor Lift, the employee was standing on the top rails of the lift to reach higher to perform welding work, exposing the employee to the hazard of falling approximately 12 feet, and possible tipover of the lift. Scaffold training shall include the design criteria, maximum intended load-carrying capacity and intended use of the scaffold.
