29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating, or moving parts of equipment exposed to contact by the employees or which otherwise created a hazard, were not guarded:     On or about August 06, 2013, employees were exposed to caught-in-between hazards, when working around the mortar mixer, which did not have a guard covering the sprocket, belts and pulleys, while employees are mixing the mortar.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:     On or about August 06, 2013, employees were exposed to falls of up to approximately 22 feet in height to the ground level below, when using a welded frame scaffold, which was not provided with a ladder, while accessing the working platform.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:    On or about August 06, 2013, employees were exposed to the hazard of falls of up to approximately 22 feet in height, to the ground level below, from a welded frame scaffold, which was not provided with a guard rail at the front and ends of the scaffold platform, while installing a cinder block wall to the addition of the building.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:     On or about August 06, 2013, employees were exposed to being struck by cinder blocks when working below a scaffold platform, which was approximately 22 feet in height to the concrete level below and not having a toeboard, while employees were installing the cinder blocks to the back wall of the building.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:      On or about August 06, 2013, employees were exposed to impalement hazards, when exposed to falls of approximately 19 and 22 feet to the concrete below, where rebar ends were protruding approximately 26 inches from the concrete floor, while employees were working from the scaffold with no guard railing at the end.
