Guardrail around the compressor was missing. The fall height into the pit was measured at approximately 4 feet 11 inches. This condition exposed employees to a fall hazard. Findings include but are not limited to: 1) The employer failed to modify or relocate the Control room building to lower the maximum predicted BLD below 2 or relocate personnel. 2) The employer failed to update the facility procedures and ensure all personnel are trained and proficient in PSM standards 3) The employer did not document inspections of the CTF respiratory protection equipment. 29 CFR 1910.26(c)(3)(i): A simple rule for setting up a ladder at the proper angle is to place the base a distance from the vertical wall equal to one-fourth the working length of the ladder: (a) Enterprise Products Operating, LP at 1250 County Road 69, Rifle, CO 81650: On and before 8/1/13, the employer did not ensure that the portable metal ladder in cooling fan and slug catcher was at the proper angle for ensure safe use. The extension ladder was not placed at the proper angle with a secure footing. It was braced against the underside of the cooling fan. Employees use the ladder to perform maintenance work on the fans and the slug catcher. This condition exposed employees to slip, trip, and fall hazards.
