29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: On or about August 06, 2013, employees were exposed to the hazard of falls of up to approximately 22 feet in height, to the concrete level below, from a welded frame scaffold which was not provided with a guard rail at the front and ends of the scaffold platforms, while installing a cinder block wall to the addition of the building.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: On or about August 06, 2013, employees were exposed to impalement hazards, when exposed to falls of approximately 19 and 22 feet to the concrete level below, where rebar ends were protruding approximately 26 inches from the concrete floor, while employees were working from the scaffold with no guard railing at the end.
