29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) PRODUCTION AREA - Employee(s) used a four receptacle outlet box that is designed to be secured to a structure, as a hanging pendant style power outlet, to provide power to production equipment, including but not limited to the Strapack JK-2 strapping machine and the Moll BC 733 counting machine, thereby exposing employee(s) to possible injuries, on or about August 6, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) MAIL DEPARTMENT - Employees plugged the power cord of the Strapack JK-2 Strap Machine (Model Number 12005245 602) where the strain relief of the plug end had been compromised, exposing approximately one (1) inch, thereby exposing employee(s) to electrical shock injuries, on or about August 6, 2013. No abatement certification or documentation required.
