29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.   Job site located at 14441 DuPont Street, Omaha, NE 68117-Employees engaged in residential construction activities (roofing) 6 feet or more above the ground level were not protected from falling with the use of a guard rail system, safety net system, or personal fall arrest system.  Employees were wearing a full body harness, but were not tied off to a safety rope with a rope grab system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.   Job site located at 14441 DuPont Street, Omaha, NE 68117-Employees working on a roof and were exposed to fall hazards of 11 to 19 feet had not received fall protection training from the employer.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder:  Job site located at 14441 DuPont Street, Omaha, NE 68117-Employees were allowed to access the working area of a roof with a portable ladder where the side rails did not extend at least 3 feet above the landing surface.  The employees were exposed to this hazard on the day of the inspection and the day after the initial inspection.
