On or about 8/6/13, workers working from a job-made platform setting the ridge pole on the two story residential home did not utilize a fall protection system and were exposed to falls of up to 12-feet, 9-inches. 2. On or about 8/6/13, workers working and walking on the second floor deck with unprotected sides and edges did not utilize a fall protection system and were exposed to falls of up to 12-feet.
