29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 123 Maple Street, Lexington, MA  1. On or about 8/6/13, workers working from a job-made platform setting the ridge pole on the two story residential home did not utilize a fall protection system and were exposed to falls of up to 12-feet, 9-inches.  2. On or about 8/6/13, workers working and walking on the second floor deck with unprotected sides and edges did not utilize a fall protection system and were exposed to falls of up to 12-feet.

29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:  Location: 123 Maple Street, Lexington, MA  On or about 8/6/13, the employer did not guard the window openings of the newly constructed house, exposing workers to fall hazards of greater than 12-feet.
