29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): The employer does not ensure that all working levels of scaffolds are fully planked. This violation was observed on or about August 7, 2013, at the job site where an employee was exposed to a fall hazard when working from the third tier of a scaffold that was not fully planked. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform on all working levels of scaffolds are fully planked.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. The employer does not install guardrails along all open sides and ends of scaffold platforms. This violation was observed on or about August 7, 2013, on the east side of the structure where an employee was exposed to a fall hazard when working from the third tier of a scaffold without guardrails. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working from scaffold platforms.
