29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a. At the site - employees were exposed to a struck by hazard, in that, the use of hardhats was not enforced. Employees were operating scissor lifts underneath steel beams and not wearing head protection. Violation observed on or about 08/07/13.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a. East side of building - employees were exposed to a 16'-6" fall hazard, in that, the use of fall protection was not enforced. Employees were installing panels and were working 2 ft from the edge. Violation observed on or about 8/7/13.
