29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On 8/6/13, employees were reroofing a two story steep pitch house while using incomplete and improper personal fall protection in that: a) Three workers were using positioning devices for fall arrest.  b) Three roof anchors had more than one lifeline connected to them.  c) Three lifelines were tied to the roof anchors and not connected with snap hooks.  d) Two employees had their lifelines hooked to their chest straps.  e) One lifeline was wrapped around the chimney and hooked back onto itself.  f) One employee tearing off shingles on the garage roof had no fall protection of any kind.
