29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   a)  Triplett Machine Co., on or about 8/7/13:  The Employer allows voluntary use of N95 filtering facepieces and 3M half-mask respirators and has not supplied employees with Appendix D of the respirator standard.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  a)  Triplett Machine Co., on or about 8/7/13:  Employer had not established nor implemented a written respiratory protection program when employees are voluntarily wearing respirators for protection when pressure testing and blasting parts.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:   a)  Triplett Machine, Inc., on or about 8/7/13:  The Employer did not provide annual training to employees on the use and application of portable fire extinguishers.    *ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy; as outlined in 29 CFR 1910.38(a) and .39(a) which includes the evacuation requirements of 29 CFR 1910.157(b).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  New Building, on or about 8/7/13:  (2) Two Sunex single spindle drill presses were not provided with guards to cover the serrated chucks and tooling.  b)  TC6 Area, on or about 8/7/13:  Hardinge lathe was not provided with a chuck guard.  c)  TC6 Area, on or about 8/7/13:  Lagun Milling machine was not provided with guards to cover the chuck and tooling.  d)  Shaft Department, on or about 8/7/13:  (3)  Bridgeport milling machines were not provided with chuck and tool guards.  e)  Shaft Department, on or about 8/7/13:  Buffalo dual spindle drill press was not provided with a guard to cover the serrated chuck and tooling.  f)  TC6 Area, on or about 8/7/13:  Hardinge speed lathes were not provided with guards over the collets and collet closer.
