At the dike area where employees used a cut off aluminum ladder to gain access to the hoses, the foot of the ladder was broken, rung bent, and the ladder was not long enough to be used on site thereby exposing employees to a fall hazard.

Each disconnecting means required by this subpart for motors and appliances shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. a. In the main electrical room of the mill, the main Siemans disconnect was not marked as to the equipment it serviced thereby exposing employees to a contact hazard. The Holmes Ag Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(f)(1), which was contained in OSHA inspection number 625599, citation number 1, item number 12 and was affirmed as a final order on (date), with respect to a workplace located at 301 S. Market Street, Holmesville, Ohio 44633.

In the main electrical room of the mill on the front of the Grain Pit disconnect, there was an unused opening to a energized electrical panel thereby exposing employees to a contact hazard. b. In the main electrical room of the mill on the front of the grain leg energized electrical panel, there was a unused opening thereby exposing employees to a contact hazard. c. In the main electrical room of the mill on the front side of the electrical panel, there was an unused opening in the energized electrical panel thereby exposing employees to a contact hazard. d. In the main electrical room of the mill on the side of the ITE Disconnect, there were three unused openings on the side of the energized electrical disconnect thereby exposing employees to a contact hazard. The Holmes Ag. Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 625599, citation number 1, item number 13 and was affirmed as a final order on (date), with respect to a workplace located at 301 S.
