The permit shall certify that the precautions contained in this paragraph (1910.272(g)) have been implemented prior to employees entering bins, silos or tanks. The permit shall be kept on file until completion of the entry operations. On or about August 7, 2013, and times prior thereto, the employer did not ensure the following:  a)  Employees had not been trained when required to enter bins to perform clean-up activities, exposing employees to oxygen-deficient atmosphere and/or explosion.  b)  Employees required to use fall protection had not been trained when personal protective equipment (PPE) was necessary, how to properly wear PPE or the proper care, maintenance, useful life and disposal of the PPE, exposing employees to fall hazards.  OR IN THE ALTERNATIVE (for fall protection):  29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment (PPE).
