Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected. a) Rapid Production Tooling, Inc. at 265 Bunyan Ave., Berthoud, CO 80513: On and before 7/18/13, the employer did not ensure that precautions were taken to prevent the ignition of flammable vapors. Employees transferred isopropanol alcohol from a metal 55 gallon drum into smaller containers for use. All items and objects involved in the transfer were not maintained at the same potential energy, in that bonding and grounding were not used to dissipate static charge. This condition exposed employees to a fire hazard. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Rapid Production Tooling, Inc. at 265 Bunyan Ave., Berthoud, CO 80513: On and before 7/18/13, the employees were not provided suitable facilities for quick drenching or flushing of the eyes within the battery charging area. An eyewash station was not provided. This condition potentially exposed employees to corrosives. Abatement Note: A feasible means of abatement includes but is not limited to: 1) a portable eye wash device, appropriate for the expected hazardous exposure, and containing not less than one gallon of potable water; or 2) a plumbed emergency eyewash unit capable of delivering not less than 1.5 liters (0.4 gallons) of clean water per minute, with flow valves that are simple to operate, go "on" in one second or less, and remain "on" during the eyewash procedure. For details on emergency eyewash and shower equipment, OSHA references the American National Standard for Emergency Eyewash and Shower Equipment, ANSI Z358.1.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 psi: a) Rapid Production Tooling, Inc. at 265 Bunyan Ave., Berthoud, CO 80513: On and before 7/18/13, the employer did not ensure the compressed air nozzle used for cleaning off equipment in the shop was reduced to less than 30 psi. The compressed air line was pressurized at approximately 80-90 psi. This condition exposed employees using the air hose to the hazard of an embolism in the blood stream.
