29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:     a) Exterior Front of Building - An employee not wearing eye protection was  observed hand mixing Lehigh Type 1-11 brick cement which contained Portland Cement, on or about 08/08/13.     b) Exterior Front of Building - An employee not wearing eye protection was observed cutting a stone paver with a DeWalt 7 and a quarter inch portable electric grinder, model number DW028493 , on or about 08/08/13.     For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.

29 CFR 1926.403(d)(1): Electric equipment was not firmly secured to the surface on which it was mounted:    a) Fourth Floor - An employee was using a heat gun plugged into a 4-way outlet laying on the floor, 110 volt, 15 amp service, on or about 08/08/13.       For the purposes of this inspection, this employer was determined to be the controlling and correcting employer of the jobsite. The employer is being cited under OSHA's Multi-Employer Workplace Policy.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
