29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and were placed in service when examination showed any condition adversely affecting the safety of the vehicle. Such examination was not made at least daily:    a) On or about August 8, 2013 and at times prior, a Toyota forklift, Model 7FGCU25 and Serial number 68127 was not examined for defects and safety prior to use. The wheels of the forklift were worn out and the warning lights were inoperable.
