This violation was observed on or about August 8, 2013, at a workplace located at 1408 Rapids Ct., Rockwall, Texas 75087; where at least one (1) employee, who was working from a job-made supported scaffold while securing rafters to the ridge beam, was not protected from falling more than 10-feet to the lower level.
