29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Fabrication Warehouse: On or about August 8, 2013, employees were exposed to a laser hazard from a Shop Sabre 7214 CNC Router equipped with a COHERENT© Diamond E-Series Class IV Laser due to the use of safety eye glasses which were not rated for the specific wavelength in use.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate: Production Warehouse: On or about August 8, 2013, employees were exposed to a struck-by hazard from material which was stored in an area above the employee breakroom that was not rated for an approved load capacity or for storage purposes.
