29 CFR 1904.7(b)(4): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent. (a) Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491: On or about 31 July 2013, the employer incorrectly logged a work-related injury onto its OSHA 300 Log for Calendar 2013 in the following instance: Instance No. 1: No case number: On 28 Jan 2013, a criminal investigator fractured the 4th metacarpal bone on his left hand, no direct cause described. This case should not have been recorded on this OSHA 300 Log but on the OSHA 300 Log for the BIA-Office of Criminal Justice also located in Whiteriver, AZ and maintained separately by that other BIA Branch. The employee is not assigned to or contracted to the Fort Apache Agency, but has a separate reporting structure. Abatement Certification and Documentation is required
