(a) Facilities Maintenance Shop, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491:  On or about 31 July 2013, and at times prior thereto, the employer did not correctly describe the object or substance tht directly injured the person noted in column (F) of the OSHA Form 300 Log for Calendar Year 2013, in this instance:  INSTANCE NO. 1:  Case number 132290287, a Maintenance Worker experienced a rash on both arms on 5/7/2013, which resulted in medical treatment.
