29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    a. In the rebay building, a large industrial fan was placed in front of the northeast door obstructing the exit.  b. In the rebay building, the northwest door was not free and unobstructed. A horizontal baler machine was placed partially in the path of the exit route.

29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted.  a. In the rebay area, the employer did not ensure the exit sign on the Northeast door was illuminated by an electrical or chemical light source. The exit sign was plain paper, broken and falling apart.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   a. In the rebay building, the employer did not develop a program to establish procedures, training and periodic inspections for employees who performed lockout/tagout on the Bollegraf horizontal baler machine when entering the pit under the machine to clean out fallen materials.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  a. In the rebay building, the TCM 495 forklift model 55 FSSSSA084, did not have a functioning horn. The TCM forklift was the only one in the facility that employees used to move recycling material throughout the day.
