29 CFR 1910.106(d)(2)(iii): Flammable or combustible liquid containers were not in accordance with Table H12: (a) Warehouse, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491: On or about 1 August 2013, two 2 gallon plastic containers of Class 3 flammable liquid (gasoline) were stored on the floor between racks of combustible material in the SWFF supply cage, exposing employees to hazards.

29 CFR 1910.141(a)(5): Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin: (a) Restroom in Warehouse, Fort Apache Agency, PO Box 560, Whiteriver, AZ 85491: On or about 1 August 2013, the lavatory room for the warehouse had holes in the walls below the sink and behind the commode, allowing rodents, insects, and other vermin to entire the restroom and workplace, exposing the employees to hazards.
