29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).   a.  On or about August 9, 2013, in the facility, employees were required and allowed to operate powered industrial vehicles on a daily basis.  The employees did not receive proper formal and practical training on the safe operation of powered industrial vehicles, thereby exposing employees to struck by and crushing hazards.

29 CFR 1904.29(a): Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  a.  On or about August 9, 2013, in the facility, the employer did not maintain OSHA 300 logs for the years of 2010, 2011, 2012 and 2013 as required.
