29 CFR 1926.100(a):   Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Jobsite located at 9 Elizabeth Avenue, Lavallette, NJ 08735:  Employees were exposed to head injuries while installing the aluminum soffit while working on the overhang of the roof of a residence under renovation.  The employees were working without using head protection, observed on or about 05/06/2013.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:  a)  Jobsite located at 9 Elizabeth Avenue, Lavallette, NJ:   Company employees from Maria Angamarca Nivelo DBA Maria Angamarca Nivelo., were exposed to fall hazards of approximately 12 feet to the ground surface below when they were using a scaffold that did not have the top guardrail while employees were installing the aluminum soffit on the residence under renovation.  Occurred on or about 05/06/2013.
